2005 JAQUAR INVITATIONAL May 6-8, 2005 T-SHIRT AND SWEATSHIRT ORDER FORM Dear Coach: The tournament T-shirt logo is available in four styles. All shirts will have the logo printed on the front and team names on the back. Pre-ordered shirts are discounted by $2.00 each. The prices on this order form represent the discounted prices. The black Hooded sweatshirts are available only on a preorder basis. This item will not be available for sale the weekend of the tournament. White short sleeve and long sleeve shirts, as well as short sleeve tie dyes will be available in limited quantities the weekend of the tournament. Please indicate how many items by size your team needs. This form must be received by mail, email, or fax (fax 716-824-1187) no later then April 21, 2005. If ordering by mail please send one check with your completed order form payable to Eagle Point Sportswear 2955 South Park Ave. Lackawanna, NY 14218. Pre-orders can be picked up and paid for at Tournament Headquarters the weekend of the tournament.
