S. Ather Enam Chair, Department of Surgery Aga Khan University Karachi, Pakistan I welcome all the participants to the Second Annual Surgical Meeting of the Aga Khan University Hospital. We have been able to expand upon last years meeting both in terms of the scale, and the scope. This year‟s theme and scientific program are focused on Surgical Innovation for the Developing Countries, an area of endless possibilities. A focused meeting such as this one is the perfect opportunity for experts from around the world to gather, and explore this area further. The workshops will allow participants substantial hands-on experience related to 17 different topics. The scientific program, much like last year, is both exciting and informative. I am thankful to all the guests who have gathered here upon our invitation. I must also thank our collaborators, and contributors, who have helped make this event possible. And finally, I must congratulate the Organizing Committee for this herculean effort. Aga Khan University in general and the Department of Surgery in particular has maintained its reputation as regional academic leaders, through the organization of this wholesome scientific meeting. S. Ather Enam MD, PhD (USA), FRCS (Ire), FRCS (Can, SN), FACS (USA) Diplomate American Board of Neurological Surgeons Professor, Neurosurgery Chair, Department of Surgery Aga Khan University, Karachi

Masood Umer Chair, Organizing Committee 2nd Annual Surgical Conference As chair of the index conference, it is with great pleasure that I welcome all the faculty and residents, especially those coming from overseas and north of Pakistan. It was in March this year that I took over this responsibility and since then we have been working very hard to give a new shape to our conference. This is a two day conference focusing on surgical innovations for the developing world. Our aim was to spread awareness about latest innovations in the surgical field. In fact our focus was their application in our developing world in a cost effective manner. Recent advances like virtual surgery, e-health, e-clinics, robotics and surgical simulation would be dealt in detail throughout the two days. We have moved away from the traditional free paper sessions; our plenary lectures have been named as orations after eminent medieval Muslim surgeons. This time we are having two such orations besides having one panel discussion on e-health and a debate on robotics. Our first day is dedicated to a total of 17 workshops. These will all be held in the newly constructed state of the art Centre for Innovation in Medical Education (CIME) at our university. These will mostly be held be hands-on skills training workshops focusing on the post graduate residents and giving them a chance for close faculty interaction. In addition, this year we have involved all our eleven sections of surgical specialties to present their own scientific programs. Eminent national and international faculty has been invited as guest speakers. Parallel to the scientific program we will have an indexed journal manuscript publication (JPMA) of the approved free papers mostly as short reports. We got an overwhelming response of getting about 274 free research papers from all over the country. Forty were carefully selected after a transparent and rigorous peer review process for final publication. The conference APP from Whova has been purchased to bring the whole conference activity on your smart phone.

Our hope is to provide our surgeons of all specialties a flavor of latest innovations which can then help them improve their clinical practice. This meeting will indeed lay the platform for more local and cost-effective innovations targeting our non-affording population. I am sure our brilliant residents will get many new and leading ideas placing them in a much better position to enter independent practice. All our workshops & scientific sessions are accredited by AACME. Department of Surgery at AKU plans to hold such mega academic events on an annual basis. Our more than 100 surgical faculty members in the department will continuously strive together to become national flagbearers in innovative surgical practice and education. One cannot conclude anything on this conference without mentioning the tremendous help and input by our secretary, Shariff Charania. He was the real backbone of this conference. At the same time, we must thank our industry sponsors for supporting us in this endeavor. Masood Umer MBBS (AKU), FCPS-Orth Associate Professor Section Head, Orthopaedic Surgery Department of Surgery Aga Khan University, Karachi

Shahzad Shamim Co-Chair, Organizing Committee 2nd Annual Surgical Conference There‟s a way to do it better - find it! These are the words of Thomas Edison, perhaps the greatest innovator in human history. After „Surgical Complications‟ last year, the theme of this year‟s Annual Surgical Meeting is „Surgical Innovations for the Developing Countries‟, inspired by innovators like Thomas Edison and Steve Jobs. The goal of the meeting is to gather current and future surgical leaders with international experts, create an environment of discourse, and provide the necessary stimulus to inspire surgical innovations despite resource restraints. This year‟s meeting has a lot more to offer than last year. The build up to the event was helped by the announcement of JPMA Supplement, which the participants will receive along with other conference material. We have also organized 17 sub-specialty workshops, some of which are the first of their kind in Pakistan. We have maintained our traditional opening ceremony, which allows an informal networking opportunity for the participants. This year we have encouraged our own medical students to develop an entertainment program for our esteemed guests. We have also introduced breakfast just ahead of the main scientific program, to allow not just an early start, but also another excuse for informal interaction between participants and faculty. The Scientific Program is the icing on the cake. There will be 11 concurrent sub-specialty sessions in the morning, featuring Pakistan‟s best and brightest surgical minds interacting with international experts, surgical trainees and students. The afternoon is dedicated to the plenary session with a carefully thought out program, incorporating theme based talks, panel discussion, keynote lectures, and finally a debate with an audience vote, as our grand finale. Steve Jobs termed „Innovation‟, as the distinguishing factor between a follower, and a leader. The 2nd Annual Surgical Meeting is indeed an attempt to create surgical leaders within us. M Shahzad Shamim MBBS, MCPS, MRCS, FCPS, FEBNS, FACS, FRCS(Eng), FRCSGlas(Neuro Surg) Director, Internship Program Director, Surgical Neuro-Oncology Fellowship Program Head of Academic Events and Communication Division Assistant Professor, Neurosurgery Department of Surgery Aga Khan University, Karachi

Amir Shariff Chair, Scientific Committee 2nd Annual Surgical Conference On behalf of the Scientific Committee I would like to welcome you to the 2nd Annual Surgery Conference at the Aga Khan University. Following last year‟s conference on the “Impact of Surgical Complications”, this year‟s theme attempts to remain true to our endeavor to bring to the fore matters relevant to our environment and context. “Surgical Innovations for the Developing World” will be the focus of this year‟s conference. Practicing in constrained environments, I am sure most of us have created out-of-the-box solutions to provide adequate care to our patients, something that we popularly refer to as “jugadh”. Almost all of these would fall into the category of “frugal innovation” a concept that is garnering much attention globally. One of possibly the greatest discoveries of this nature involves the invention of the stethoscope. The French physician Rene Laennec was called to examine a young woman, but too embarrassed to put his ear to her chest, he used a sheaf of paper rolled into a cylinder to auscultate the heart*. Stemming from frugal innovation comes “reverse innovation” where ideas developed in resource constrained environments are used as cost effect alternatives in high income environments. We will be commencing with workshops on October 21st followed by the inaugural ceremony. October 22nd will be split into two halves, with specialty based sessions in the morning, followed by multidisciplinary plenary sessions in the afternoon. A diverse group of international and national experts have been brought together to share their thoughts and experiences and to generate debate and perhaps some controversy! We hope that this will be not just an informative and interactive meeting but one that will also stimulate and inspire. We look forward to hosting you at the 2nd Annual Surgery Conference at the Aga Khan University. Amir H. Shariff MBBS, Diplomate American Board of Surgery Assistant Professor, General Surgery Department of Surgery Aga Khan University, Karachi *Fayssoil, A. René Laennec (1781-1826) and the invention of the stethoscope. Am J Cardiol. 2009 Sep 1;104(5):743-4.

Abu al-Qasim al-Zahrawi, known also by his Latin name Albucasis, was born near Cordoba in 936 CE. Considered to be the father of surgery, his outstanding contribution to medicine is his encyclopaedic work Al-Tasrif li-man 'ajaza 'an al-ta'lif, a long and detailed work in thirty treatises. The book contains the earliest pictures of surgical instruments in history. of all the contents of Al-Zahrawi's Al-Tasrif, book 30 on surgery became the most famous and had by far the widest and the greatest influence. In it, he developed all aspects of surgery and its various branches, from ophthalmology and diseases of the ear, nose, and throat, surgery of the head and neck, to general surgery, obstetrics and gynaecology. Military medicine, urology, and orthopaedic surgery were also included. Expressing his credo in this book he states: "Whatever I know, I owe solely to my assiduous reading of books of the ancients, to my desire to understand them and to appropriate this science; then I have added the observation and experience of my whole life."
Ibn Sina, also known as Avicenna (c. 980 – June 1037) was a Persian polymath who is regarded as one of the most significant thinkers and writers of the Islamic Golden Age. Known as the father of Medicine, his most famous works are The Book of Healing – a philosophical and scientific encyclopedia, and The Canon of Medicine – a medical encyclopediawhich became a standard medical text at many medieval universities and remained in use as late as 1650. Besides philosophy and medicine, Avicenna's corpus includes writings on astronomy, alchemy, geography and geology, psychology, Islamic theology, logic, mathematics, physics and poetry.
