FOCUS A giant thromboembolus captive in a patent foramen ovale

A 69-year-old man was admitted to our department presenting with an acute pulmonary thromboembolism and a snake-shaped embolus captive in the patent foramen ovale (PFO) extending from the right atrium through the left atrium into the mitral valve ostium and the left ventricle (fig. 1A, B). An emergency operation was indicated due to the morphology of the thromboembolus and the fact that its localization carried a high risk of both systemic and pulmonary embolism. Surgical extraction of the thromboembolus on cardio-pulmonary bypass followed (fig. 1C, D).
The postoperative course was complicated by serious right heart failure resulting from another massive pulmonary embolism. A pulmonary embolectomy (Trendelenburg operation) was performed but the patient died due to irreversible right heart failure. The autopsy confirmed the presence of a massive, fresh pulmonary embolism.
