ANT TO KNOW ONE OF the best/worst-kept secrets about breastfeeding? How difficult it can be to get the hang of it. Sure, it’s natural. And yes, it’s best for babies. And absolutely, it offers significant health benefits to moms. Yet of the 78 percent of Canadian moms who initiated breastfeeding in 2009, only 22 percent were still doing it six months later. Significant numbers abandon breastfeeding within the first month, in fact. These statistics show Canadian breastfeeding rates fall well below globally recommended standards. Why are so many of us opting out of breastfeeding? One likely culprit is the unexpectedly steep learning curve for babies and moms alike. Many of us believe a copious milk supply and baby’s ability to form a good latch come “naturally” (they don’t!) and if success isn’t immediate, something’s wrong. Concerned our milk supply is inadequate or that baby isn’t getting enough nutrients, many of us switch to formula. Other times, painful nipples drive us to the bottle—unnecessarily. The most important thing to remember? Don’t panic! These challenges are all normal, surmountable and worth overcoming. One of the most common refrains we hear from breastfeeding moms is: “Breastfeeding’s one of the most challenging things I’ve ever done—but also one of the most rewarding.” So how can we make the process smoother so more new moms can make it past the many land mines of the first couple of months? Here are our top six secrets to staying motivated.

Keep your eye on the prize. Make a list of the health benefits breastfeeding provides to your baby, now
and for the long term. Here are just a few: Breastfeeding ♦Reduces infant gastrointestinal illness by up to 82% ♦Reduces respiratory illness by up to 30% ♦Reduces the risk of sudden infant death syndrome (SIDS) by up to 500% ♦Reduces the risk of ear infections by up to 300% ♦Lowers the risk of childhood obesity by up to 35% and diabetes by up to 40% ♦Protects against chronic diseases like Celiac disease, inflammatory bowel disease, asthma and childhood cancer ♦Improves neural development by up to 10%. Post your list where you can see it every day. Breastfeeding isn’t the only option out there, but it’s the gold standard for infant nutrition. For every week you breastfeed, you confer health benefits to your child.

Remind yourself of how you benefit too. Women who breastfeed improve their long-term health by ♦Reducing their breast cancer risk by as much as 25% ♦Reducing ovarian cancer risk by as much as 20% ♦Reducing osteoporosis risk by as much as 400%. In the short term, studies have shown breastfeeding moms experience less postpartum depression and anxiety. And hello pre-baby wardrobe: Breastfeeding burns about 600 extra calories per day. Many of the contents of human milk are pulled directly from our internal stores, a.k.a. “baby fat.”

Don’t compare apples to oranges. Or breast milk to formula. They’re nutritionally different, and digested differently by infants. Breastfed babies ingest smaller quantities more frequently and have �R CANADIANFAMILY.CA 11
{feeding} runny, neutral-smelling bowel movements. Formula fed babies eat more less often and have fully formed, fouler-smelling bowel movements. Breastfeeding babies are often less chubby than their formula-fed peers; they aren’t “skinny,” but they have a greater proportion of lean muscle. The best way to gauge your infant’s growth is to ask yourself if he’s happy, active and growing. Unsure? Talk with your doctor. Meanwhile, using the World Health Organization’s Growth Standards charts is the best way to determine if your baby’s flourishing or not. They describe the optimal growth for infants and children, as opposed to conventional charts which may offer average weights (skewed by formula feeding) that may place breastfeeding infants on the smaller side of things when they’re actually right on track.

If you’re having trouble, make an appointment with a lactation consultant. Why can’t a newborn just latch onto your breast the way a baby monkey does to its mom on a TV nature show? It’s one of life’s great mysteries. But a lactation consultant can help you and your baby find a good fit. They can also answer your questions relating to nutrition, holds, milk supply, mastitis and other issues that may be coming between you and breastfeeding success.

Invest in a good pump and bottles. There will be times when you feel like you’re about to burst…but your baby’s fast asleep. The right pump will help you express milk without any discomfort and especially without pain. Portable models allow moms to pump dis-
creetly at home or at work. Choose bottles carefully to avoid “nipple confusion.” New research shows that when an infant nurses they create a vacuum with muscles in their tongue, jaw and mouth. Milk flows from the breast only when baby sucks; when an infant pauses to swallow or take a breath, milk flow stops completely. Not so with conventional bottles. Milk may flow even when baby isn’t sucking, so he or she has to thrust their tongue forward to slow the milk flow. In the short term, babies have latching difficulties until they figure out how to suck in each context. In the long term, exclusive bottle-feeding can lead to dental malocclusion. (Translation: Start saving for your kid’s future orthodontic bills.) Fortunately, nipple confusion can be avoided with a natural-feeling nipple on a bottle that prevents milk flow completely unless baby sucks.

Get help. Not everyone can provide hands-on breastfeeding advice or support. That said, there are ways your friends and relatives can help out. Ask your partner to firmly tell doubters not to secondguess your breastfeeding decision. Let visiting relatives bottle-feed expressed breast milk. They’ll be delighted to get involved…and you can nap or go for a walk. If visitors ask how they can help, tell them. Seriously—if they’re asking, they want to help but don’t know how. We moms work miracles every day, starting from gestation and birth, through to the ability to generate the perfect food for our infants. You can do it. You already have, so just keep going.

f your babe’s eyes are locked on your fork at each meal, it may be a sign she’s ready for solids. The Canadian Paediatric Society (CPS) suggests waiting until a baby is six months old before introducing simple solids. However, says Henry Ukpeh, MD, clinical assistant professor of pediatrics at the University of British Columbia and a pediatrician in Trail, B.C., that age is not written in stone. FOUR OR SIX MONTHS?
“Some people argue for four months, some for six, but it is anytime in between,” says Dr. Ukpeh. If your baby is happily breastfeeding or bottle-feeding, there is no need to rush, he says. At six months, though, your baby will need the nutrition, tastes and textures of solid foods, plus breast milk or formula until she is 12 months old. LOOKING FOR SIGNS
Your baby may be ready for solids if she ♦Is hungry for feedings earlier than usual or still hungry after ♦Can sit up without support and has good control of neck muscles ♦Holds food in her mouth without pushing it out. FOOD ALLERGIES
Worried about food allergies and the early introduction of solids? It is usually only a concern if you have a family history of seasonal and food allergies, says Dr. Ukpeh.

active ingredient in various name brands, including Tylenol, Tempra and Panadol. Ibuprofen is found in Advil and Motrin. The CPS recommends parents use these drugs only for pain or fever greater than 38.5˚C. If your baby has a fever, consult a doctor before administering any medication. Babies under three months with a fever should be taken to a doctor immediately. Do not give children products with acetylsalicylic acid (such as Aspirin) as it has been associated with Reye’s syndrome, a sickness that can cause brain and liver damage. ♦RASH CREAMS Michael Rieder, MD, a professor of clinical pharmacology at Western University in London, Ont., recommends a simple barrier cream such as zinc oxide to help provide diaper rash relief. If your baby has eczema, a dry, itchy skin condition, he says parents can use a cream with 0.5 percent hydrocortisone, a very mild steroid used to treat inflammation. ♦ANTIBIOTIC OINTMENTS These topical creams help keep minor wounds or stitches from developing infection. If your baby

is allergic to any antibiotic you should avoid drugs of the same class (e.g., if he is allergic to one sulphonamide, avoid all sulphonamides). If in doubt, ask your physician or pharmacist. ♦ANTIHISTAMINE Dr. Rieder says if you need to use an antihistamine to ease allergy symptoms (itching), a liquid is preferable to allow for a more accurate dosing. ♦SALINE DROPS Salt water drops help soften mucus and can be helpful for babies whose stuffy noses make feeding and breathing difficult. Use drops that contain saline only, says Leigh Anne Newhook, MD, a pediatrician with Janeway Children’s Health and Rehabilitation Centre in St. John’s. She recommends following up with a hand-held nasal aspirator to suck out the mucus. ♦ELECTROLYTE SOLUTION Dr. Newhook also recommends that new parents have an oral rehydration solution on hand in case of gastroenteritis, in order to prevent dehydration due to excessive vomiting or diarrhea. The solution is a mixture of water, salts and sugar, and can be bought at most drugstores.

OUR POOR BABE has a full, rumbling belly and her cry is one of discomfort— all signs she’s suffering from gas. So what can you do? ♦SLOW DOWN Babies swallow air every time they eat, “especially when (they) gulp quickly with large milk letdowns,” explains Henry Ukpeh, MD, a pediatrician from Trail, B.C. Ensuring your baby has a good latch can help reduce the air gulped in, and pausing the feed for a few minutes to burp her in an upright position can relieve gas, says Amanda Spakowski, a Toronto birth doula. ♦TWEAK YOUR DIET Onions, garlic or dairy products in a mother’s diet can trigger gas in nursing babies. For parents who are formula feeding, dairy proteins can be difficult for some babies to digest. Spakowski suggests a lactose-free, goat’s milk or soy formula, but talk to your doctor before making the switch. ♦MAGIC TOUCH Abdominal massage can relieve gas and help with digestion, says Spakowski. “With your baby on her back, place your fingers firmly on the bottom left side of her belly, run your fingers up her side to the bottom of her rib cage, across to the top right of her belly and down to the top of her leg.” A firm downward rub on her back or raising your baby’s knees to her chest and stretching the legs back out again can also help move gas. ♦JUST A STAGE As your baby’s digestive system begins to mature, gas will become less of a problem. If she appears ill, refuses to eat or drink or has any other symptoms, like fever, vomiting or distension (tightening) of the abdomen, see your doctor. CANADIANFAMILY.CA 23

suspected sleep training sounded like a ridiculously overreaching parenting concept: Who the heck needs to be trained to sleep?! Then you had a baby. Sleep training is the process of helping baby learn to fall asleep on her own—and to stay asleep, hopefully through the night… or at least long enough so you don’t die of exhaustion. To get the top sleep training tips we spoke with Tracey Ruiz, The Sleep Doula, who has helped more than 2,500 families get the sleep they need.
QUESTION: Does sleep training
have to start ASAP if you want your baby to learn to sleep on their own? ANSWER: There’s no crucial window, but both parent and child have to be ready. I find children to be most receptive between five and seven months. It’s easier to begin before your child can stand up.
screaming—in that case, she’s starting to think about self-soothing. Normally, within three to five days of starting sleep training you should see a difference in your child’s sleep. If you don’t, she may not be ready.
QUESTION: What are the best ways to lull a baby to sleep? ANSWER: In addition to the usual white noise machine and physical support, a transitional item like a lovey can be very helpful. It has to be age-appropriate, though. For babies aged four or five months, a cloth breast pad is nice, not a blanket—that’s unsafe. Also, allow your child to find their way from one sleep cycle to the next. A lot of times, we hear babies grunting in their sleep and we think: “Uh oh, they’re waking up!” We go feed them, instead of waiting. Waiting lets them grunt and moan and sometimes put themselves back to sleep. Another tip is, from very early on, break the feed-sleep association by doing a diaper change or another activity right before laying them down. It can make the transition to independent sleep easier later on. QUESTION: Any final advice for stressed out moms and dads? ANSWER: Do what you have to do to survive—as long as you’re doing it safely. It doesn’t have to be forever. Children handle change much better than we do.
QUESTION: What are some signs that you’re trying too soon? ANSWER: Your baby may not be ready if she is not showing signs of self-soothing behaviour, like leg banging or hand sucking. If I’m working with a family and the baby does nothing other than flip out, she’s probably not ready, unlike a child who periodically sucks her hand and then starts CANADIANFAMILY.CA 29

Seats purchased in the United States have different safety standards, so, by law, they cannot be used in Canada. Exercise caution if buying a used car seat or one from an unknown seller online. The seat could have been in an accident or have missing pieces. Health Canada recommends a child stay rear-facing until they reach the seat’s height and weight restrictions. But even if
the provincial law says you can go forwardfacing, rear-facing position is safer. If you’re unsure how to properly install your seat, some companies provide this service for a fee (check your local listings). Organizations like St. John Ambulance (sja.ca) and municipal police associations also offer regular car seat safety information sessions and free installation clinics. or gripe water work sometimes, but not all the time. Probiotics have recently been used with some success. ♦TRY SOME GOOD VIBRATIONS. Motion, such as rocking, walking or a car ride, may help relieve distress. ♦DON’T SWITCH FORMULAS. Changing to a soy or lactose-free formula is not recommended without asking your doctor first. ♦POP IN A PACIFIER. It’s called a soother, after all. Non-nutritive sucking may relieve symptoms. ♦RULE OUT OTHER PROBLEMS. Gastroesophageal Reflux Disease (GERD) can resemble colic. Usually, the pain or discomfort of GERD is associated with constant spitting up after most feedings and occasional vomiting. It can be treated with prescription antacids.

it’s how they express their needs. But if you’ve shared a cuddle and tended to hunger and a soiled diaper and the crying continues, something’s up. That something may be colic. “There’s a big difference between a colicky baby and a fussy one,” says Moshe Ipp, MD, a pediatrician at The Hospital for Sick Children in Toronto. “All babies fuss, but it lasts for five, 10, 15 minutes, it’s erratic and it usually subsides with comforting.” Colic often involves daily bouts of inconsolable crying that can last for hours at a time. It can be accompanied by clenched fists or drawn up legs and the passage of gas, and tends to occur at predictable times, such as the early evening (the witching hour) for some unknown reason. There are many theories about what causes colic, but none have been proven. Suspected factors include the swallowing of air, an immature bowel, a milk allergy or intolerance and infant temperament. While it’s unclear what causes colic, what’s reassuring is that it’s a normal occurrence and babies grow out of it. “Colic symptoms begin at about two or three weeks and subside by three to five months of age,” explains Dr. Ipp. In the meantime, here are a few tips for colic relief.

and coordinator of the community and child studies foundation program at Centennial College in Toronto, bristles at the suggestion you can spoil a baby. “No way,” she says. “It’s developmentally impossible.” In fact, she says, it’s essential that parents respond to their baby’s cries. “It creates a trust that allows them to venture away from us later on because they know that if they need us, we’ll be there,” she says. “If you don’t respond, there’s a risk they will become clingy because you haven’t established that trust.”

That said, there is one caveat: It’s important to let your baby learn how to self-soothe. “Parents don’t want to be unavailable to their babies, so as soon as they wake and fuss, they get picked up or fed,” says Peter Nieman, MD, a community pediatrician in Calgary. With a newborn, it’s important to respond right away, Dr. Nieman says, but once a baby becomes more self-aware, you’re better to back off a bit. He suggests this stage can occur as early as three or four months. “I encourage parents to gradually wait a
little longer as they age because babies do develop the ability to self-soothe and can condition themselves to go back to sleep without always needing to be fed or cuddled.”

One clue your baby is becoming more self-aware is when it becomes clear he’s getting the hang of cause and effect, says Brandon. Around that time, gradually stop reacting as quickly to his demands so he has a chance to figure things out on his own, she says. This is especially important when it comes to food. “Sometimes when a baby fusses, the easiest thing is to settle them with food, but I encourage parents not to do that,” Dr. Nieman says. “Because if you do it often enough, it will become a habit and they will expect it—and demand it— every time.”

When it comes to showering him with toys, remember that your baby wants you more than anything else, adds Brandon. “As they grow out of that newborn stage, it’s all about interaction anyway,” she says. “It doesn’t need to be complicated or expensive—just singing a song or playing a little game, like peekaboo, is often all it takes to make them happy.”
