Bilingual Health Promoter – Community Health Promotion and Early Years Term contract of 6 months - working full-time The Health Promoter is responsible for working with community members both on-site and in the community within the Centretown Community Health Centre (CCHC) catchment area, on issues that affect their health and well-being, The Health Promoter works with the community members of the interdisciplinary Community Health Promotion and Early Years team to conduct and/or facilitate the planning, development, implementation and evaluation of health promotion programs and activities, within a determinants of health framework. Health Promotion strategies employed include: strengthening community action; building healthy public policy; creating supportive environments; developing personal skills and re-orienting health services. Requirements: Essential              

Master level degree in health or social sciences or equivalent combination of education and experience. Minimum of three (3) years’ experience as community health educator/health promoter. Specific knowledge related to motivational interviewing, behaviour change, tobacco cessation, mindfulness based stress reduction. Experience facilitating community groups and utilizing adult learning techniques. Ability to work well with marginalized groups. Knowledge of health promotion principles, and experience with a range of health promotion strategies (eg .community organizing, advocacy for public policy). Familiar with data collection methods and evaluation. Ability to work well independently and as part of multidisciplinary team. Knowledge and experience using Electronic Medical Records. Intermediate to advanced computer skills (MS Word, Outlook, Excel and internet). Ability to work flexible hours, including evenings and weekends. Organizational skills including time management. Sound judgement and strong problem solving skills. Excellent written and verbal communication skills.

Desirable     

The ability to communicate in a language other than English or French. Training Enhancement in Applied Cessation Counselling and Health (TEACH) certification or willingness to attain certification. Chronic Disease Self-Management Program “Better Choices, Better Health®” certification or willingness to attain certification. Knowledge and experience in food security. Health promotion experience with physical activity and seniors.
