Break Thru is doing a survey on sport for kids with a disability and we would love your opinion! What is the survey about? We want to find out why children with disabilities don’t participate in mainstream sport. Your feedback will help us change this situation so All Kids Can Play sport! You can also go in the draw to win an iPad mini! We are interested in your opinion on schools and sport clubs. There are two surveys to choose from, or you can do both!  You can do the survey about schools here: http://svy.mk/1bQVEDi  You can do the survey about sport clubs here: http://svy.mk/18uQNow What do I have to do? Simply click on the links above. By doing this you agree to voluntarily take part in the survey. The survey is anonymous and takes about 10 minutes to complete. There are no right or wrong answers. You can stop being involved at any time by closing the page. Who can do the survey? We want to hear from as many people as possible! The survey can be completed by people involved in sporting activities at your local school or sport club, people with disabilities, principals, teachers, coaches, volunteers, children and parents. Feel free to send the survey to any of your friends who may be interested in this issue. I’m under 18. Can I participate? Break Thru is interested in the views of people of all ages, so we have designed the survey to include a section for children and young people. Your parent/guardian must complete this survey with you to demonstrate consent to your participation. Who can I contact for more information?
