Community Referral Program Information Packet Through this telephonic health education and case management assistance service, we provide continuity of care that enhances patient outcomes and health literacy. Following a referral submitted by your office, our highly trained staff calls the patient and works to meet their needs by providing education, navigation, counseling, and social support. This program can also collect and report non-identifying data back to the provider, helping reach best practices and meet outcome goals.
