* Level of excision—skin lesion: In the 2010 CPT code book, paragraphical notes above code 11400 and code 11600 state that these codes [11400–11646] classify the “[e]xcision (including simple closure) of benign/malignant lesions of skin. ... Excision is defined as full-thickness (through the dermis) removal of a lesion, including margins, and includes simple (non-layered) closure when performed ... Repair by intermediate or complex closure should be reported separately.” * Subcutaneous soft tissue tumors: In the 2010 CPT code book, a paragraphical note above code 20000 states: “Excision of subcutaneous soft tissue tumors (including simple or intermediate repair) involves the simple or marginal resection of tumors confined to the subcutaneous tissue below the skin but above the deep fascia.” * Fascial or subfascial soft tissue tumors: In the 2010 CPT code book, a paragraphical note above code 20000 states: “Excision of fascial or subfascial soft tissue tumors (including simple or intermediate repair) involves the resection of tumors confined to the tissue within or below the deep fascia, but not involving the bone.” * Radical resection of soft tissue tumors: In the 2010 CPT code book, a paragraphical note above code 20000 states: “Radical resection of soft tissue tumors (including simple or intermediate repair) involves the resection of the tumor with wide margins of normal tissue.”
