Credential Booklet Instructions

NOTE: DO NOT LEAVE THE TOURNAMENT WITHOUT RETREIVING YOUR CREDENTIAL BINDER. TURN IT IN TO YOUR DIRECTOR OR THE COQUILLE OFFICE. Credentials in manila folders or similar items will not be accepted per the State Commissioner. No exceptions. Start with a ¾ or 1-inch size 3 ring binder, white in color. The binder should be the type with a clear plastic cover over the top forming a pocket on the front and back. Inside the 3 ring binder you will need; 1. 20 letter size sheet protectors for 3 ring binders. 2. 3 business card size sheet protectors for 3 ring binders. You can get all of these items at an Office Depot, Staples or similar type of store. Using Microsoft Word or similar program you need to create a cover sheet with; 1. 2. 3. 4. 5.

Place the cover sheet inside the clear plastic pocket on the front of the 3 ring binder. You can repeat for the back cover as well. In the sheet protectors you will need the following items; Page: 1 Front – (LOE) Letter Of Eligibility – Printed from the document center of our league Babe Ruth website. Back - Any letter of waiver you may have. – Most will not have one. Page: 2 Front – Certificate of Insurance – Printed from the document center of our league Babe Ruth website. Back – Current League Charter Certificate – Printed from the document center of our league Babe Ruth website. Back – Copy of this year’s rec. league schedule for your age group. – Contact your age group commissioner Pages: 4, 5 and 6 (Business Card Size Sheet Protectors) Front Only – Babe Ruth Player I.D. Cards For Every Player. (Absolutely No Birth Certificates Allowed) Player I.D. Pages: 7, 8, 9 & 10 Front Only – Consent for treatment forms. - Printed from the document center of our league Babe Ruth website. (see link above) 4 originals per player each with original signatures. No photo copies. Page: 11 Front Only – Color team photo with all players and coaches. All players and coaches must be in the same uniform. Minimum size of 5x7 and must be able to easily identify all persons in the photo. Can be digital that is copied and pasted into a Word Document. Players and coaches must be identified left to right and front row to back. Example: “Row 1 Kneeling from left to right; Player Name, Player Name, etc. Row 2 Standing from left to right; Player Name, Player Name, etc. Last Row Standing from left to right; Coach Bob Smith, Coach Billy Smith, Coach Ben Smith” Pages: 12 & 13 Front and Back – Coaching Certification Certificates All coaches must take and pass the Babe Ruth Coaching Certification Course. http://www.baberuthcoaching.org
