STATEWIDE HEALTH INSURANCE ENROLLMENT STARTS The Maryland Health Benefit Exchange is scheduled to open for business in October, with benefits taking effect in January 2014. The statewide goal is to make health insurance affordable and accessible for all Maryland residents, including the approximately 730,000 residents who are currently uninsured. Holy Cross Hospital is partnering with the Montgomery County Department of Health and Human Services to provide education, outreach, eligibility and enrollment services for the nearly 222,000 uninsured residents of the Capital Region in Montgomery and Prince George’s Counties. CONSTRUCTION UPDATE

Just 14 months ago, we began construction for the first new hospital in Montgomery County in 35 years. When Holy Cross Germantown Hospital opens in the fall of 2014, it will serve the most rapidly growing and aging region in the county with medical, surgical, obstetric, emergency and psychiatric care.

Construction at Holy Cross Hospital also is progressing. Our new seven-story, 150-bed patient care building is on schedule to open in 2015. We have completed the first phase of the project— expansion of the parking garage—which offers 300 additional parking spaces.

JOY Advanced Back and Neck Surgeries Relieve a Lifetime of Suffering After four decades of pain, Joy Lazaroff finally had had enough. Joy, now 54, had injured her back when she was 14 years old. Then, six years later, a car accident fractured vertebrae in her neck. Over time, her conditions got worse, to the point where the pain would leave her bedridden for weeks. Joy had tried conservative treatment—such as medication and physical therapy, but without success. She knew that she would have to have two surgeries someday—one to repair her back and a separate one to repair her neck—but she was scared. “I put it off for over 20 years,” Joy says. “Finally, when I couldn’t get off the couch or stand up straight any more, I was ready to consider back surgery.” Joy’s reluctance was understandable. She had heard for years that back surgery didn’t work—or made the problem worse.

“If I had known before what I know now,” says Joy Lazaroff of Silver Spring, “I would have done the surgeries much sooner.” 4 Holy Cross Health Today

ADVANCED TREATMENT OPTIONS Philip Schneider, MD, orthopedic spine surgeon and medical director of the Holy Cross Hospital Spine Center, has heard many similar stories. “I mostly see people who have had problems for a long time,” Dr. Schneider says. “When I saw Joy, she had tried everything and there was really no other option except surgery.

IS PAIN FREE I told her, ‘If you’re ready, we can fix it.’ Nowadays, back and neck surgeries have a very high success rate.” An expert in minimally invasive spinal surgery, Dr. Schneider trains physicians from around the world in cutting-edge spinal surgery through the GATE Institute at Holy Cross Hospital, a training center for physicians who perform minimally invasive procedures. For Joy’s back surgery, Dr. Schneider teamed with Frederic Schwartz, MD, neurosurgeon, Holy Cross Hospital. Joy had spinal stenosis, a narrowing of the opening of the spine, which causes nerves to be crimped. Dr. Schwartz opened up the spinal canal to relieve pressure on the nerves. Joy also had an unstable spine because her bones were misaligned. Dr. Schneider performed a spinal fusion using the least invasive approach available, placing rods and screws to stabilize and strengthen her spine. This resulted in a faster recovery because there is less blood loss and smaller incisions. Joy was only in the hospital for two days. From the time she arrived at Holy Cross Hospital’s Spine Center, Joy noticed a dramatic difference from previous stays at other hospitals. “I had a private room that was not just comfortable for me, but also for my visitors,” she says. “Plus, there were no restrictions on visiting hours. Room service brought meals when I wanted them. And the laminate wood floors made it easy to walk with a walker.” The nursing staff in the Spine Center are specially trained to take care of patients with spine surgery

and the center enjoys very high patient satisfaction rates. Within just four weeks after surgery, Joy was walking tall and straight. In eight weeks, she was back at work, and after 12 weeks, she resumed her fitness walking regimen.

MINIMALLY INVASIVE NECK SURGERY Joy’s back surgery went so well that she felt comfortable talking to Dr. Schneider about her neck pain. She was losing feeling in one hand and could barely turn her head. An MRI showed herniated disks in her neck. At first, Dr. Schneider tried nonsurgical treatments, but her pain was becoming more severe. A second

MRI confirmed that her condition was clearly getting worse, and Dr. Schneider recommended proceeding with surgery. Using minimally invasive techniques, Dr. Schneider fused three disks and inserted a metal plate and screws in her neck. “This was all done through a very small incision in the front of the neck, which means less pain and no bleeding,” he says. “Joy was able to turn her head immediately after surgery.” Joy’s neck surgery required just a single night in the hospital. Today, for the first time since she was a teenager, she’s essentially pain-free. “Two years ago, I couldn’t stand up straight. I can’t believe I’m the same person,” says Joy. “Dr. Schneider changed my life.”

Brain Surgery AFTER A STROKE, TINY COILS SAVE BEANIE RYAN’S LIFE Beanie Ryan of Burtonsville doesn’t remember having a stroke, but her family will never forget that day in early May. They knew immediately that something was very wrong. “About 4:30 in the morning, she started kicking me in the back,” Beanie’s husband, Kelly, recalls. “She was moaning and I thought it was a bad dream. I tried to wake her up, but she was not responding. Our daughter called 911. The emergency medical technicians rushed to get her to the hospital.” When she arrived at Holy Cross Hospital, Beanie was confused and drowsy, with slurred, incoherent speech. On a stroke grading scale of 1 to 5 (5 is just before death), she was at grade 3. “Usually with grade 3, 50

percent of patients will die or have a severe stroke,” says James Jaffe, MD, medical director, Interventional Neuroradiology, Holy Cross Hospital. CT scans showed that Beanie, then age 62, had suffered a hemorrhagic stroke, which occurs when an artery on or near the surface of the brain bursts and bleeds into the brain. In Beanie’s case, the bleeding was due to a ruptured aneurysm. A hemorrhagic stroke is much less common—and more devastating—than an ischemic stroke, which is caused by a blood clot. Fifty percent of patients with hemorrhagic stroke die before reaching the hospital. Only about 25 percent of patients with a ruptured aneurysm will regain full function. Holy Cross Hospital specializes in minimally invasive procedures for stroke patients that allow physicians to treat conditions in the brain without making an incision in the skull. As an interventional neuroradiologist,

Other stroke symptoms include sudden trouble with seeing in one or both eyes, difficulty walking, dizziness, loss of balance or coordination, or a sudden severe headache with no known cause. 6 Holy Cross Health Today

During an endovascular coiling procedure, the coils fit into the shape of the aneurysm and are meant to form clots so that it won’t bleed again.

Dr. Jaffe uses X-rays to guide catheters into the brain to either block bleeding aneurysms or remove blood clots. For Beanie, Dr. Jaffe performed an endovascular coiling procedure. Through a 1/4" nick in the groin, he inserted a catheter to navigate to the blood vessels to the brain. Contrast dye was injected into the brain to determine the kind and shape of the aneurysm. Then a smaller catheter was used to deliver and place soft platinum coils, into the aneurysm. The coils fit into the shape of the aneurysm and are meant to form clots so that it won’t bleed again. Coiling is only the first step in treating a hemorrhagic stroke; monitoring is just as important. Blood that leaks into the brain can be irritating to other blood vessels and cause vasospasms, which narrow the blood vessels in the brain, resulting in decreased blood flow.

“This means there is a risk of patients suffering strokes for several days after surgery,” Dr. Jaffe says. “Undetected vasospasms can be deadly if they are not monitored properly.” That’s why Holy Cross Hospital’s Neuro Critical Care Unit (NCCU) is so important. The unit serves brain and spine patients and those with ischemic and hemorrhagic strokes.

“We have an extraordinarily dedicated team of medical professionals working in the NCCU,” says Zachary Levine, MD, medical director, Neurosurgery, Holy Cross Hospital. “Critical care physicians are on duty 24 hours a day, 365 days a year and they work along with the unit’s specially trained nurses, nurse practitioners and physician assistants

who provide continuous patient assessment and monitoring.” Beanie spent 11 days in the NCCU. “It was the best care I’ve ever had,” she says. “I had the best nurses. I could not thank them enough.” Just three weeks after what could have been a fatal stroke, Beanie was able to be with her grandson to celebrate his first birthday.
