Dear Mr. Yopp: ATC Associates Inc. (ATC) performed a FDIC Environmental Checklist for the above-referenced property. The property is an approximately 846-square-foot single-family residence located on approximately 0.72acres of land. The structure is 1-story with a basement and a detached garage. The exterior of the structure is brick. The remaining lot is manicured grass with concrete walkways. The property building was reportedly constructed in 1954 and is currently vacant. The physical address for the property is 3502 W. 58th St, City of Chicago, Cook County, Illinois 60627. The property is identified as Property Identification Number (PIN) 19-14-214-040-0000. The survey was performed on October 3, 2012 without access limitations. Attached, please find the completed FDIC Environmental Checklist. The deliverable includes (1) a completed Initial Selection Form, (2) Completed Individual Field Checklists if indicated on Initial Selection Form, (3) photographs of the property and (4) other supporting documentation, if any. It is our professional opinion that there are existing environmental conditions at the property with regard to environmental hazards, based on our limited site visit and review. Based on the inspection, review of additional documentation, and the age of the property, the following is recommended for further investigation: x

The property is recommended for a Comprehensive Asbestos Survey. An asbestos survey has not been performed on this property. A comprehensive asbestos survey is required to identify and sample suspect building materials and determine their condition and the presence of asbestos. The primary objective of a comprehensive survey is to identify the location, the quantities, and the condition of all asbestos materials. If desired or required, a cost estimate for the removal of the asbestos materials can be made from the results of or as part of a comprehensive survey. An abatement cost estimate cannot be made from data derived in a preliminary asbestos survey. If any renovation or demolition work is planned for this property, an asbestos survey needs to be implemented immediately.

The property is recommended for a Phase I ESA or Lead-based Paint Inspection/Survey. Test the paint to determine if it has a lead content above the applicable legally permissible threshold for leadbased paint in housing.
