This symposium - Going Beyond Mechanical Thinking in Cancer Surgery – is designed to educate participants on a new innovative therapeutic approach, PIPAC (Pressurized IntraPeritoneal Aerosol Chemotherapy). For one century, cancer surgery has been based on simple mechanical principles: open, remove the tumor and close. Nowadays cancer surgery is performed in a closed environment that can be controlled and steered to improve patient’s outcome. This is the principle of PIPAC – and an emerging field of research with major therapeutic potential. Time has come to integrate the opportunities offered by physics, chemistry, biology and pharmacy into the operating field. For this purpose and to meet the evolving challenges in providing optimal therapies to our cancer patients, surgeons and gynecologists must collaborate with engineers, pharmacologists, biologists and chemists. Convergence between disciplines, disease sites, and treatment approaches will shape the future of cancer surgery. Our ability to strengthen the delivery of and access to PIPAC technology will depend on the quality of the scientific evidence on disease and therapy. PIPAC is not only an exciting drug delivery technique, but also provides us with powerful methodological tools. For example, thus far there is no accepted standard allowing therapy response assessment in minimal-volume peritoneal disease. An international group of distinguished pathologists have now developed the Peritoneal Grading Regression Score (PRGS) that will be presented at the symposium. Therapeutic aspects will also be covered during the meeting. Six clinical trials on PIPAC are ongoing in various indications and several further trials are under regulatory approval. Present and futures studies will be presented, including a pivotal controlled trial in platinum-resistant, recurrent ovarian cancer to be started in 2017.
