Health Partners Limited Optical Makeover Promotion Terms and Conditions 1. These Terms and Conditions apply to Health Partners “Optical Makeover” Promotion (“Promotion”). By taking part, entrants agree to be bound by these conditions. Claims must comply with these conditions to be valid. 2. The Promoter is Health Partners Limited ABN 43 128 282 904 of Level 3, 101 Pirie Street, Adelaide 5000 SA. Phone: 1300 113 113 (“Promoter”). 3. The Promotion commences at 11.00 am (ACST) on 1 February 2017 and closes at 9.00am (ACST) on 20 February 2017 (“Promotion Period”). Entries after 9.00am (ACST) on 20 February 2017 will not be eligible for the Promotion. 4. Prize consists of two (2) style makeovers in total valued at AUD $900 and two (2) pairs of optical frames up to the value of AUD $400 each (1 pair of frames for each winner). This Promotion consists of a total of 2 x style makeovers and 2 x optical frames (“Prize”). 5. Entry into this Promotion is open to all Health Partners Members who are over the age of 18 years and current financial policy holders (“Eligible Entrants”). 6. Limit of one (1) entry into the promotion per person. Entry is via a registration form on the Health Partners website. Valid entries must have all fields completed correctly. 7. Incomplete, inaccurate or ineligible entries, or entries not completed in accordance with these Terms and Conditions will be deemed invalid. The Promoter’s decision is final and binding and no correspondence will be entered into. The Promoter reserves the right to disqualify any entrants that provide false information. 8. Proof of identity, residency, Health Partners membership and eligibility is at the discretion of the Promoter. In the event that an entrant cannot provide sufficient proof, they may forfeit the Promotion in whole and no substitute will be offered. 9. Eligible Winners who meet the Entry Requirements will be entitled to receive the prize, unless they notify the Promoter that they do not wish to participate in the Promotion. 10. Two winners (1 x male & 1 x female) will be chosen at random at 12.00pm (ACST) on 20 February 2017 at 101 Pirie Street, Adelaide SA 5000. Winners will be notified via phone or email on 21 February 2017. 11. The makeover will occur on a date and at a location to be confirmed by Health Partners. The Prize (Optical frames) will be provided to the winners once the makeover is complete.

12. Once the Prize has been accepted, the Promoter holds no further responsibility for the Prize and no substitute prize or monetary recompense will be provided. 13. Except for any liability that cannot be excluded by law, the Promoter (including its officers, employees and agents) excludes all liability (including negligence) for any personal injury; or any loss or damage (including loss of opportunity); whether direct, indirect, special or consequential, arising in any way out of the Promotion, including, but not limited to, where arising out of the following: (a) any technical difficulties or equipment malfunction (whether or not under the Promoter’s control); (b) any theft, unauthorised access or third party interference; (c) any entry or prize claim that is late, lost, altered, damaged or misdirected (whether or not after their receipt by the Promoter) due to any reason beyond the reasonable control of the Promoter; (d) any variation in prize value to that stated in these conditions of entry; (e) any tax liability incurred by a winner or entrant; (f) redemption of the prize; and/or (g) participation in the Promotion. Nothing in these conditions of entry restricts, excludes, modifies or purports to restrict, exclude or modify any statutory consumer rights under any applicable law including the Competition and Consumer Act 2010 (Cth). 14. In the event of war, terrorism, state of emergency or disaster, the Promoter reserves the right (subject to reference to all relevant state and territory regulations), to cancel, terminate, modify or suspend the Promotion. 15. If, for any reason, this competition is not capable of running as planned, including infection by computer virus, tampering, unauthorised intervention, fraud, technical failures or any other causes beyond the control of the Promoter, which corrupt or affect the administration security, fairness, integrity or proper conduct of this competition, the Promoter assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorised access to, or alteration of entries. 16. The Promoter reserves the right to withdraw or substitute all or part of this Promotion with another offer of equal or greater value at the Promoter’s sole discretion. Eligible Entrants will not be entitled to any additional compensation in the event that the prize or element of the prize has been substituted at equal or greater value. 17. The Promotion is not transferable or exchangeable. 18. By taking part in this promotion you consent to Health Partners taking photographs of you and using those photographs for all marketing and promotional purposes, including in print and electronic media, on the Internet and on Social Media platforms. You acknowledge and agree that Health Partners will own copyright in all images and photographs and in all material incorporating the photographs. 19. Winners of this promotion must complete and sign a Health Partners Photography Consent Form and through signing the form, release and forever discharge Health Partners and its agents, officers and employees, from any and all

claims and demands arising out of or in connection with the use of said photographs including, but not limited to, any claims for financial compensation.
