Dear friends of Isabella, 2012 was a challenging year for Isabella. As with any challenge, there are always opportunities for positive change and growth. While diminished revenue caused us to reduce some staff positions during the year, we added new services to meet growing community needs as well as improved existing programs. We are ever thankful to our dedicated and caring staff who work together in good and bad times. Nursing home based services were expanded by increasing our capacity to care for those needing short-stay rehabilitation or ventilator-dependent care. The ability of staff to communicate effectively and problem solve was measurably increased by training in coaching communication presented in partnership with PHI. This vital training will extend to all Isabella staff and was made possible through a generous grant provided by the United Hospital Fund, the New York Community Trust and the Fan Fox & Leslie R. Samuels Foundation.
Isabella Visiting Care, Inc. our licensed home care services agency has more than doubled its size in the past two years. In addition we have provided professional home health aide training to nearly 600 community residents in the past four years — all at no cost to the trainee. Innovation continues to be our watchword. In a time of increased pressure on health care providers to do more with less due to cutbacks in government-funded programs such as Medicare and Medicaid, the imperative to do well becomes ever greater. This is a challenge Isabella takes seriously and more than ever, we need your support. As you read these pages, think about what efforts cost, our community’s loss were they to be eliminated and what you can do right now. Please consider Isabella as you plan your charitable giving. Technology continues to progress as the tool of communication and innovation. During this past year Isabella modernized operations by converting from the paper-bound world of yesteryear to the electronic system of today. We participated in the creation of the platform that will enable electronic health records to transfer seamlessly to other medical providers. Our staff electronically records resident care and treatment interventions. Telemedicine remains ever more important as a means for electronically communicating vital information from our home care clients. And a new Physician Portal facilitates the timely authorization of care by community doctors. With the help of Local 1199 SEIU, all nursing home staff members have completed training to master Microsoft Office skills.

Person centered care is at the forefront of how we deliver care in our nursing home and across our spectrum of services. We pioneered in this effort more than 15 years ago and in 2012 we benefitted from several measures that strengthen our effort in these areas: • With the help of PHI, a national non-profit organization that provides training in coaching and supervision, Isabella staff received specialized guidance in how best to communicate and manage in a variety of challenging situations. This training – which continues in 2013 –enables staff to handle situations at the point of service in the best way possible as well as to communicate most effectively with residents, family members and each other. • We continued to engage family members and our community partners in helping to create a dynamic setting for those in our care. New York Cares, Inc. and Arts and Minds are just two of our valued community partners that have contributed time and creative talent to enrich the lives of our residents. It would be impossible to count the many hours that family members and caregivers have contributed to helping care for residents in our nursing home – as well as those in our community based programs. • Isabella’s own staff members have continued the tradition of finding new and festive ways to engage the residents in activities that enrich daily life. Using the theme of an “Ocean Cruise”, Isabella hosted “Summer Fest”, an annual day-long extravaganza of musical performances, games and culinary delights to entertain and engage residents. • Personal pride was very much on display as residents delighted in showing off their creative talents in Isabella’s Annual Art Show.

Here are some highlights:
Education remains an important element of Isabella’s mission. It enables our staff to remain current with the latest treatment modalities; it helps nursing home residents recover and return safely to home; and it provides guidance to community residents so that they can minimize the need for costly medical care in the future. Our commitment to education also provides incentives to the youth of the community. Here are some of our educational accomplishments this year. • Nearly 150 community residents received free career training to become home health aides and/or certified nursing assistants. • More than 400 community residents attended one or more of the 14 educational programs conducted by the Institute for Older Adults. • More than 200 community residents attended the Annual Isabella Health Fair where they learned about the services of 35 community providers. • 60 Community residents attended an Education Day on Advance Directives. • 10 Community High School seniors with a record of academic achievement and a commitment to community service were awarded Scholarships to help with the cost of college.

Our new program of Platinum Care provides additional services to complement special lifestyle and social needs.

Isabella is fortunate to have participated in several vital community partnerships in 2012 to promote important community goals: • Under the aegis of the United Hospital Fund, and with other community providers, Isabella is participating in “Together on Diabetes” a unique program for seniors with diabetes that provides education, support, community referrals and individual guidance to enable them to remain healthy and better manage their disease. • In partnership with GrowNYC, Isabella offers YUM Fresh Food for Northern Manhattan, a healthy food access program led by a coordinated committee of community senior volunteers. YUM is part of a national demonstration project – “Healthy Communities though Healthy Food” – managed by United Together on Diabetes Neighborhood Houses of New York. • Isabella provides leadership to the Upper Manhattan Partnership for Senior Independence, a consortium of 8 community organizations that offer case management services and program referrals to 900 residents of Northern Manhattan.
Extraordinary developments are rarely anticipated – but can be a wonderful source of accomplishment and pride. • Superstorm Sandy presented unprecedented challenges for Isabella. While our location above sea level presented no fear of flooding, mass transit shut downs, gas shortages and power outages wreaked havoc on staffing schedules. Thanks to the enduring commitment of Isabella staff who remained on duty until relief arrived, we were able to maintain resident care throughout the emergency. In addition, we were able to accommodate the transfer of residents from a nursing home in Brooklyn that suffered extensive damage from the storm. • In 2012 Isabella was pleased to be awarded a 5 star rating “Much Above Average” by the Centers for Medicare and Medicaid Services, a federal agency that administers Medicare and Medicaid.

Healthcare will continue to undergo major transformation in the next few years. While we must prepare for change, one thing remains constant: our commitment to improve the quality of care for all. • Short Stay Rehabilitation will continue to be an important need in our community. In 2013 we will expand our capacity to meet this need. • Our existing Ventilator Dependent Unit is fully utilized at all times and demand for this specialized care has exceeded our ability to accommodate all requests. To remedy the situation, we plan to expand capacity in 2013. • Home Care will be needed more than ever as the demand for case management and quality care in the community rises. To prepare for this, we will open a new certified home health agency “Isabella Care At Home”. • Preventing unnecessary hospital readmissions – and controlling costs – are important health care goals in 2013. In partnership with New York Presbyterian Hospital, Weill Cornell Medical Center, Allen Hospital and St. Luke’s Roosevelt Hospital, Isabella

will offer “Bridge to Home”, a free 30-day program for Medicare fee-for-service patients that will provide guidance and support before and after discharge to enable them to connect with community health care providers, secure appropriate social services and learn to properly manage their illness. • The transition to Managed Care is well underway for our community based clients. In anticipation of the upcoming requirement for Managed Care participation of all nursing home residents, we will be educating staff, family members and residents as to the changes that are anticipated. • Quality of Care will continue to be at the forefront. We will continue the training created by PHI to advance the communication and coaching skills of Isabella staff. • As part of a grant awarded to the New York Chapter of the Alzheimer’s Association, Isabella will be working to develop innovative programs for nursing home residents with dementia, including palliative care. • We will remain steadfast in our goal to maintain financial stability, quality care for all and a willingness to adapt to positive change.

We are grateful for the generosity of the many entities that make this a reality. Our Fall Annual Gala is a generous source of philanthropic funds for Isabella’s programs. Hundreds of guests gathered at the Tribeca Rooftop for an evening of dinner and dancing under the stars. This year’s Gala honored two extraordinarily dedicated individuals in the field of health care: Robert G. Newman, MD, President Emeritus of Continuum Health Partners, Inc. and a Professor of Community Medicine at Mount Sinai School of Medicine was honored for his vision and leadership in developing partnerships to meet the growing healthcare needs of all New Yorkers. Jo Ivey Boufford, MD, President of The New York Academy of Medicine and Professor of Public Service, Health Policy and Management at the Robert F. Wagner Graduate School of Public Service was honored for her contributions to the field of public health - both on a national and international level - and for her expertise in advancing the contemporary study of public health issues. Isabella held its fourth fundraiser for community based programs in early November. Called “A Night in the Heights”, this festive event, attended by more than 100 guests, offered wine tasting, a silent and live auction and spirited entertainment.
