JERICHO CULTURAL ARTS 2012-2013 INFORMATION GENERAL INFORMATION: -The program is only offered to Jericho UFSD residents. -The program will take place at all three elementary schools from 3:10 p.m. – 4:55 p.m. All courses and students’ schedules are subject to space availability and final registration. Additional sections may be added to some classes depending on staff and space availability. -Children will be supervised and will have an assigned place to be after dismissal, prior to the beginning of Session 1 courses. All students will then be escorted to their 3:10 p.m. class. At 3:55 p.m. children will be escorted to their Session 2 class or to their designated dismissal area. -Dismissal procedures and final schedules will be distributed prior to the start of the program. REFUNDS: No refunds will be made. NO EXCEPTIONS! TRANSPORTATION: There is no transportation home at 4:00 p.m. or 4:55 p.m. Parents are required to provide transportation home after the 4:00 p.m. dismissal (if needed) and after the 4:55 p.m. dismissal. Bus transportation for registered SCOPE students will be provided to SCOPE at 4:55 p.m. REGISTRATION FEES: Registration Fee: $15 per child Cost Per 12-Week Course: $100 REGISTRATION: 1. Applications are received either by mail or drop-off at the school. No email applications will be accepted. 2. Complete one registration form for each child attending. 3. Checks for registration fees must accompany applications. Make checks payable to Jericho U.F.S.D. Your cancelled check will be your receipt. Receipts are available upon request. 4.
