Online Proofing System Instructions The Wiley Online Proofing System allows proof reviewers to review PDF proofs, mark corrections, respond to queries, upload replacement figures, and submit these changes directly from the locally saved PDF proof.

1. For the best experience reviewing your proof in the Wiley Online Proofing System ensure you are connected to the internet. This will allow the PDF proof to connect to the central Wiley Online Proofing System server. USING e-ANNOTATION TOOLS FOR ELECTRONIC PROOF CORRECTION Required software to e-Annotate PDFs: Adobe Acrobat Professional or Adobe Reader (version 11 or above). (Note that this document uses screenshots from Adobe Reader DC.) The latest version of Acrobat Reader can be downloaded for free at: http://get.adobe.com/reader/ Once you have Acrobat Reader open on your computer, click on the Comment tab (right-hand panel or under the Tools menu). This will open up a ribbon panel at the top of the document. Using a tool will place a comment in the right-hand panel. The tools you will use for annotating your proof are shown below:

1. Replace (Ins) Tool – for replacing text.

2. Strikethrough (Del) Tool – for deleting text.

Fill in any details and then click on the proof where you’d like the stamp to appear. (Where a proof is to be approved as it is, this would normally be on the first page).

7. Drawing Markups Tools – for drawing shapes, lines, and freeform annotations on proofs and commenting on these marks. Allows shapes, lines, and freeform annotations to be drawn on proofs and for comments to be made on these marks.

How to use it: 

Click on one of the shapes in the Drawing Markups section.



Click on the proof at the relevant point and draw the selected shape with the cursor.



To add a comment to the drawn shape, right-click on shape and select Open Pop-up Note.



Type any text in the red box that appears.

Editor, read with interest the study recently published in Acta Ophthalmologica by Yin & Gong (2017) which evaluated meibomian gland and other clinical characteristics in patients with adult blepharokeratoconjunctivitis (BKC). I agree that readers should be reminded of the importance of care and attention to the meibomian glands

in patients presenting with BKC. One possible interpretation is that the BKC cases were patients with more severe meibomian gland dysfunction (MGD) in a spectrum of patients with EDE, whereas the comparison group had milder MGD. More severe MGD is related to increased Staphylococcus (Zhang et al. 2017), and lipid esters in meibum can induce proliferation of Staphylococcus (Shine et al. 1993). In any case, Staphylococcal exotoxin A may expectedly increase, which can then induce a hypersensitivity reaction and consequently, the marginal vascularisation and cornea infiltrates/ulceration (Ficker et al. 1989; Callegan et al. 1994). However, two points in the paper need clarification. BKC episodes, as illustrated in fig. 1 of the paper (Yin & Gong 2017), are fairly common acute presentations in many younger adults with MGD. As the BKC episodes are very inflammatory and respond to topical steroids, several aspects of tear function would be expected to change during the evolution of the disease. Therefore, the duration between the last episode of BKC exacerbation and the examination should be specified; in addition, the type of anti-inflammatory treatment and duration of treatment should also be provided. In table 2 (Yin & Gong 2017), it was unclear how the meibomian gland dropout in the BKC group could exceed 100% in the temporal part of the eyelid. A reader may assume 100%

atrophy to represent total loss of all the meibomian glands.
