Primary purpose of this position The position of Integrated Care Support: HealthPathways provides support to the Murray HealthPathways team to ensure delivery of HealthPathways according to the project timeframes. Key responsibilities Supports the Clinical Editor and Integrated Care Consultant and other members of the team to gather non-clinical information from specialty departments and other health and community services that includes, but is not limited to: • How to refer • Outpatient assessment including forms, contact details, criteria, and any information required for accurate triage • Waiting times • Where diagnostic tests (i.e., laboratory or radiology) can be accessed, referral criteria, and forms. • Other health provider services, criteria, and access details including different modalities • Will need to work with GPs, allied health, specialists, regional and rural health staff • Provide administration and logistic support (including room bookings, catering and registrations) for programs and events which are organised and run by the Murray HealthPathways team • Ensures health services have appropriate access to HealthPathways either as an individual and/or at the organisational level. Provide general administrative assistance as necessary: • Takes minutes of Murray HealthPathways meetings including work groups • Prepares and distributes minutes of the meeting following approval, • Ensures timely payment of individuals participating in clinical work groups. • Collects and enters data to ensure monthly reports are accurate and timely, • Updates CRM systems to ensure key stakeholders details are correct. • Updates DOT at the end of each working day. Other responsibilities • Actively participate in Murray PHN committees and working groups. • Work in accordance to and support Murray PHN strategic objectives as directed.

• Work in a safe manner, adhering to all work health and safety (WH&S) requirements and report all hazards and incidents through the organisation’s WH&S processes. • Comply with organisational policies and procedures including the Code of Conduct and instrument of delegation. • Actively manage risks by complying with organisational policies and procedures and escalating risks for higher-level attention when required. Note: Some travel may be required as part of this role. Note: this position will require a current driver’s licence which must be provided prior to commencing employment.

Specialist expertise • A certificate IV in office administration or business and/ or at least 2 years’ experience in a similar role of providing reception and administration services. • Experience in the health sector organisational and project management type skills will be valued Knowledge and skills (Some at Level A and most at Level B) • Verbal communication – Clearly explains information and listens to feedback. Murray PHN spans the northern region of Victoria. While the majority of the catchment area is located within the State of Victoria, it also includes the twin city of Albury/Wodonga. The estimated resident population is 565,000 people. It is estimated that the Murray PHN area includes: • 715 General Practitioners • 44 acute facilities • 200+ pharmacies • 150+ residential aged care facilities • 9 Primary Care Partnerships • 20 Local Government Authorities There are 31 PHNs (Primary Health Networks) across Australia. PHNs will work with the acute health care sector and other health service providers and communities to: • To increase the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and • To improve the coordination of care to ensure patients receive the right care in the right place at the right time. PHNs are not-for-profit organisations funded predominantly by the Australian Government.
