Questions & Answers: If submitting by electronic health record (ERH) does a practice or individual still need to use the EIDM account? The short answer to this is that it depends. It is important for practices to clarify what your vendor is reporting on your behalf and what you need to report. It is highly unlikely that the vendor will take care of everything. Practices need to discuss reporting with their vendor and be prepared to report data for themselves. Has anyone received an error message that their account is blocked in the reporting portal? Contact the QPP help desk by phone or email for any issues related to the reporting portal. The phone contact is 866-288-8292, E-mail [email protected] Do Alternative Payment Model (APM) participants have to report their Advancing Care Information (ACI) measures? Participants in APMs and Advanced APMs should check with their APM entity to determine what they need to report. Many groups will need to enter their own ACI measures. Can you submit different time-periods for different categories? Yes

Do practices or clinicians that are reporting through their EHR need to log into the EIDM portal and do anything? It depends on your vendor. Do not make any assumptions that your vendor will take care of everything contact them and confirm. In many cases you will have to upload your own files. Even if your vendor does enter data on your behalf you can use your EIDM account to check on what has been submitted. When reporting for improvement activities there is only one date range for all activities. Is it okay if a practice or clinician performed different improvement activities for different time periods? If your improvement activities have different performance periods that is okay. You will enter the earliest start date and the latest end date. This could mean that your date range is the entire year. Example: A clinician performs the following improvement activities (IA) during 2017: • IA_BE_22: performed February 1 – May 15 • IA_BE_4: performed July 1 – December 31 • IA_BE_5: performed October 1 – December 31 In this example the clinician or practice would indicate a performance period of February 1, 2017 – December 31, 2017. How can a clinician or practice determine which categories have data entered and whether these categories are combined appropriately? The best guidance for this is to log into the Merit-Based Incentive Payment System submission portal and confirm that there scores for each category. Once you log into the portal use the navigation bar on the left side of the screen and select the appropriate taxpayer identification number (TIN) associated with your EIDM login information. You will see for that specific TIN whether or not you have a score for each category and an overall performance score.
