Transcutaneous PO2 (tcPO2) monitoring (A.Fluch, 1977) is widely used in neonatal-care units,allo wing serial measurements to be performed with good reliability, no discanfort nor ethical problems, either in patients with respiratory problems and in normals. We studied the modification of several oxygenation parameters used in lung physiology, during air and 30% 02 breathing,in 6 normal newborns and in 10 with transient neonatal tachypnoea (T.T.N.). The extent of 02-desaturation was expressed by: Pa02; 0 venous admixture; alveolar-arterial O2 difference, P(A-a)02;arteri al-alveolar 0 2 ratio, P(a/A)02; alveolar-arterial difference versus inspired 02, P(A-a)02/FI02; arterial-inspired O2 ratio, (Pa/FI)02; alveolar-af: terial ductance for 02, (PI-PA)/(PI-Pa)O2. In air tcPOZ were higher than 6Ommag in 5/10 m n e w b o r n s and 70mmag or less resulted in 3/6 normals: thus in 50% the oxygen therapy could not be decided solely on the basis of PO2 values. A clearcutdif ference existed between TIN and normal newborns, when calculating the % venous admixture in air (P
