Spring 2017 CSF APPLICATION PROCEDURES Membership Period is Wednesday Feb. 1 through Wednesday, Feb. 15 ONLY 1. Fill out the application, participation AND tutoring forms COMPLETELY. All forms are REQUIRED, including the tutoring form in this packet. DO NOT SUBMIT AN APPLICATION THAT DOES NOT TOTAL 10 POINTS! Remember: the participation form requires a PARENT SIGNATURE! 2. If possible, make a clear copy of your last semester’s report card (transcripts or ParentConnect printouts are NOT accepted) to include with the application. (Do not submit the original report card.) If any grades were changed by teachers, you must submit a copy of the grade change form. By including the report card, you will earn 2 points toward active membership. Y ou may still apply without the report card. 3. Go to whscsf.org. Click “Pay for Membership” and you will be directed to the West High webstore where you will pay $7 for 2nd semester membership. Please print the confirmation email you receive after you pay and include it with the rest of the app. 4. Staple ALL items (application, participation form, tutoring form, report card, and confirmation email). D O NOT PUT APPLICATION IN AN ENVELOPE. PLACE STAPLED APPLICATION IN THE BOX IN ROOM 4108. 5. To qualify for CSF membership this semester, follow these guidelines: 1. You must earn a minimum of 10 points from last semester’s grades. a. The first 4 points must be from LIST I (unless you are a senior applying for membership in February or June). b. The first 7 points (including the 4 described in “a”) must be from LIST I and LIST II. c. The remaining points may come from any LIST (I, II, or III) 2.
You may use no more than 5 courses to qualify.
3. No CSF points are given for physical education, courses taken in lieu of physical education, summer school courses, subjects repeated to improve a grade, courses involving clerking and office/teaching assisting, and courses taken on a pass/fail basis or through independent study. 4. CSF points are granted as follows: Grade of A =3 CSF points. Grade of B =1 CSF point. Note: One additional point shall be granted for a grade of A or B in an AP, IB, or Honors course, up to a maximum of two such points per semester (e.g., an A in World History AP earns 4 CSF points). These classes are underlined on the back of this application. Grade of C = 0 CSF points. Grade of D or F in any course, even in one you cannot use to qualify, disqualifies you from membership at this time. In Fall only: Grades which were raised because of a CST incentive may not be factored into eligibility. You may only use the grades on your original Spring report card. 5. You must not have any U’s on the report card (for either the quarter or semester grade) or any instances of cheating, suspension, or other academic dishonesty.
