Hearing Loss Prevention in Cisplatin-Induced Ototoxicity in Paediatrics Author: Mukovhe Phanguphangu Department of Audiology, West Rand District Health Services, Merafong, South Africa, Audiologist, Researcher and Director, Williams Hearing and Balance Centre, Carletonville, South Africa, Department of Audiology, Pietersburg Provincial Academic Hospital, Polokwane, South Africa, Division of Communication Sciences and Disorders, Faculty of Health, University of Cape Town, Cape Town, South Africa

Background

Methods

 The cochlea can protect itself from many toxins,

however, current research indicates that 75100%

of

paediatric

patients

treated

with

cisplatin acquire cisplatin-induced ototoxicity.

Results

 To report on the current trends in an effort to

prevent ototoxicity  To stimulate research in the area of preventative audiology regarding cisplatin induced ototoxicity.

 Cisplatin, a chemotherapeutic drug, alters the endo-cochlear potential resulting in damage to the organ of corti, lateral wall and spiral ganglia.  Consequently this causes a disruption in the firing of the hair cells in the cochlea, and thus result in a sensory hearing loss and tinnitus.  Although monitoring, altering and changing dosage can be attempted, at times it is impractical to change the dose and as a result, a preventative agent would be more suited.

Conclusions

3 oto-protectors are found in literature.

 Sodium

Rybak et al (2010) adds that an ideal otoprotector should not: be toxic, be in high concentration to prevent damage to the cochlea and not interfere with the chemotherapeutic actions of cisplatin.
