How do I access Patient 

~ My Concord Hospital Stay?

When you provide your e-mail address to Concord Hospital you will receive an e-mail, following your discharge, with instructions for registering your Patient Connect ~ My Concord Hospital Stay account. To register your account, follow the instructions provided in the e-mail. If you are unable to register your account, or have difficulty using the portal, call RelayHealth Customer Support at 1-866-RELAY-ME (1-866-735-2963) or e-mail [email protected].

I no longer have the e-mail invitation sent to me following my discharge. How do I register my account without it? If you no longer have your e-mail invitation and want to register your account you may contact RelayHealth by visiting https://app.relayhealth.com, e-mailing a request to [email protected], or calling 1-866-RELAY-ME (1-866-735-2963).

Where do I find my discharge instructions? From the portal’s homepage click the Health Records menu and then click the Files tab. You will then click ‘View’ to view information about your hospitalization including your discharge instructions.

Yes, Patient Connect ~ My Concord Hospital Stay is powered by RelayHealth, a secure online service. RelayHealth uses encryption technologies to protect the privacy and confidentiality of your personal health information.

What if I forget or lose my password? If you forget or lose your password, you can reset it by visiting https://app.relayhealth.com and clicking on the ‘I forgot my user ID or password’ link. You may also call RelayHealth Customer Support at 1-866-RELAY-ME (1-866-735-2963) or send an e-mail to [email protected].

What if I did not receive an e-mail invitation from Concord Hospital asking me to register my account? If you did not receive an e-mail invitation from Concord Hospital, please check in your e-mail accounts junk mail and spam folder. If you do not find the invitation in either of those folders, contact RelayHealth Customer Support at 1-866-RELAY-ME (1-866-735-2963) or e-mail [email protected].

How do I cancel my Patient 

~ My Concord Hospital Stay access?

We believe accessing information documented during your hospitalization empowers you to take an active role in your health. However, if you choose to cancel your access you can do so through the portal’s account settings and clicking on ‘Cancel Account’. Upon cancellation, you can still log in to your account but will have restricted access and Concord Hospital will no longer send RelayHealth information about your Hospital stays.

If I am currently a Concord Hospital Medical Group (CHMG) patient that has access to Patient  ~ My CHMG Health Care, the outpatient portal, do I need to sign up again? Yes. If you are a Concord Hospital Medical Group patient using Patient Connect ~ My CHMG Health Care, you must register your Patient Connect ~ My Concord Hospital Stay account to access information related to your hospitalization. Using Patient CONNECT ~ My Concord Hospital Stay When will my medical information be available in Patient  Hospital Stay?

Your personal information will be available in Patient Connect ~ My Concord Hospital Stay two hours after your discharge from Concord Hospital.

Can I use the same password for my Patient  ~ My Concord Hospital Stay ~ My CHMG Health Care account? account that I do for my Patient  Yes, you can use the same password.

Can I access my child’s record and/or a dependent that I am a legal guardian of? You may add family members to your My Concord Hospital Stay portal at your discretion. However, at this time you will not have access to information on their recent Concord Hospital stay. We hope to add this feature in the future.

Do I need to be 18 years old to access my Patient  Stay account?

Yes. Only patients 18 years of age or older will be asked to provide an e-mail address upon registration.
