Primary purpose of this position The primary purpose of the Integrated Care Consultant is to support change and improvement with the team who are working across the primary health care system in order to contribute to improving health outcomes in the region. The position leads the coordination and implementation of PHN projects at the regional level with a particular focus on working with health providers to strengthen local service delivery particularly with primary care settings. The role is a key interface with the key position leads from the Strategy, Planning & Commissioning team and Health Integration & Engagement teams to ensure horizontal and vertical dialogue occurs to meet Murray PHN’s identified priorities and related strategies. Key responsibilities With the support of the Executive Director Regional and as part of a whole of organisation approach to effective engagement with providers, key responsibilities are to: • coordinate a project management approach to the planning, support and evaluation of Murray PHN projects. • identify and optimise opportunities that support a shared vision for change with stakeholders and reflect organisational values. • work with others to initiate and maintain effective relationships with key personnel within health providers in order to support: o provider readiness and performance o clinical engagement o organisational understanding o information and knowledge exchange • draw upon organisational resources and specialist support to initiate, coordinate and implement health system improvements across the region. • encourage effective work practices and relationships that generate collaboration, innovation and efficiencies across the organisation. • coordinate the team’s effort to meet organisational information management and governance needs, specifically for compliance, budget and performance reporting requirements.

Other responsibilities • Actively participate in Murray PHN committees and working groups. • Work in accordance to and support Murray PHN strategic objectives as directed. • Work in a safe manner, adhering to all work health and safety (WH&S) requirements and report all hazards and incidents through the organisation’s WH&S processes. • Comply with organisational policies and procedures including the Code of Conduct and instrument of delegation. • Actively manage risks by complying with organisational policies and procedures and escalating risks for higher-level attention when required. Note: Some travel may be required as part of this role. Note: this position will require a current driver’s licence which must be provided prior to commencing employment.

Specialist expertise • A degree qualification in a health-related field combined with more than 5 years’ experience in a similar role. Background on Murray PHN
Murray PHN spans the northern region of Victoria. While the majority of the catchment area is located within the State of Victoria, it also includes the twin city of Albury/Wodonga. The estimated resident population is 565,000 people. It is estimated that the Murray PHN area includes: • 715 General Practitioners • 44 acute facilities • 200+ pharmacies • 150+ residential aged care facilities • 9 Primary Care Partnerships • 20 Local Government Authorities There are 31 PHNs (Primary Health Networks) across Australia. PHNs will work with the acute health care sector and other health service providers and communities to: • To increase the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and • To improve the coordination of care to ensure patients receive the right care in the right place at the right time. PHNs are not-for-profit organisations funded predominantly by the Australian Government.
