The Program/Concentration Change Request Form allows you (the student) to request changes to your intended degree at Trident University International. Trident requires a new copy of this form if you would like to: • • •

switch program/concentration, or declare multiple concentrations, or seek readmission to a program/concentration no longer offered on the application.

If you would like to switch to a new program/concentration, please select your new desired program/concentration by marking ONE of the boxes below. If you would like to declare multiple concentrations or if you are seeking readmission to a program/concentration no longer offered, please mark the OTHER box under the appropriate college and degree level, and type in your desired program/concentration(s). All Active Duty, National Guard, or Reserve Soldiers utilizing Army Tuition Assistance, as well as any civilian personnel using the GoArmyEd portal, must first process any changes through www.goarmyed.com before submitting this form. For a list of program/concentration codes, please contact your advisor. Please note that program or concentration changes may result in new catalog rights and/or additional course requirements. Previous transfer credits applied to your degree are subject to review.
