SmileConnect® Advancing Children’s Oral Health Tooth decay is the most common chronic disease affecting children today, with children from the lowest socioeconomic groups experiencing issues at significantly higher rates and at younger ages. With support from the Centers for Medicare & Medicaid Services (CMS), Altarum is leading systems change innovation to improve coordination and delivery of oral health care across medical, dental, social service, and school-based settings, ensuring children receive the preventive care they need in order to reduce adverse outcomes associated with untreated dental caries. SmileConnect® is a comprehensive oral health care coordination solution that promotes evidence-based oral health practices by providing enabling technology support to coordinate existing resources, and activate care coordinators and community providers to promote and sustain preventive oral health care for their pediatric patients.
SmileConnect® Community is a social networking web application (SmileConnect.org/Community) that leverages philanthropic donations of dental supplies and existing skilled workforce to fulfill the need for oral health education and preventive services among children in their local communities. Dental providers, and dental and dental hygiene students, often volunteer their valuable resources for education and preventive services. To streamline the process of making these local connections, SmileConnect® Community allows educators and community organizations to easily post requests for oral health resources and services, which are made available to those who can directly connect with them to meet the need. SmileConnect® Community is currently active in 30 states directly connecting nearly 100,000 kids with needed resources.
SmileConnect® Clinical is a dental referral management application that increases communication and transparency between medical, dental, and social service communities allowing for better coordination in patient treatment and referrals across these settings, positively impacting the quality of oral health care provided, and increasing early prevention among young patients. SmileConnect® Clinical is accessible through both MILogin (the State of Michigan’s Single Sign On) and Allscripts Professional, and is in the process of integrating with additional Electronic Health Record systems.
SmileConnect® Continuing Medical Education (CME) offers comprehensive live in-person and remote training, technical assistance, and Smiles for Life certification to primary care clinicians and clinical support staff, aligned with the American Academy of Pediatrics recommendations to implement preventive oral health services during well-child visits. Through a joint-providership with Michigan Medicine, Altarum offers 30-50 CME credits and 20-25 Maintenance of Certification (MOC) Part IV credits to eligible clinicians who complete two cycles of Plan-Do-Study-Act data guided quality improvement over a period of seven months. Participating clinics significantly increased their rates of preventive oral health service provision, improving at a rate of 60-80% above the pre-training baseline period (see graph).
A pediatric oral health registry, SmileConnect® Reporting is a CMS Meaningful Use Specialized Registry, enabling eligible providers who submit data to achieve Meaningful Use. SmileConnect® Reporting houses valuable information related to a child’s access to and utilization of preventive oral health services in both the medical and dental environments, and currently there are more than 1,500 physicians and dentists that have attested to use the system.
