eAppendix 1 – Patient Survey This survey instrument was administered using RedCap and invitations were sent via secure Patient Gateway email. Introduction Our records indicate that you recently viewed your condition and procedure list in Patient Gateway. Viewing this list is a new feature in Patient Gateway, and we are conducting a survey to learn more about how helpful the list is to patients. Taking the survey is optional, and your responses will be kept confidential. If you choose not to answer this survey, the health care you receive will not be affected in any way. Your responses will only be accessible to the research team, and will only be reported anonymously with responses from other patients.
