Background Currently, the gold standard for determining hydration status involves central venous catheters, an invasive procedure that carries significant risks including mechanical complications and infection. Focused US of the inferior vena cava (IVC) and heart correlate strongly with a patient’s volume status and have been used to guide resuscitation measures.
Study Objective The purpose of this study is to evaluate EM resident ability to interpret focused IVC US in order to assess hydration status.
Methods This was a prospective study of residents from two ACGME-accredited EM residency programs in the Northeast region of the United States. Residents were asked to review twenty IVC US studies demonstrating various levels of patient hydration. Prior to being reviewed, all US clips were de-identified and evaluated by two US fellowship-trained, ARDMS-certified, EM attendings. After viewing each IVC US clip, residents were asked to diagnose the patient as euvolemic, hypovolemic, or hypervolemic. Performance was compared to year of training (PGY1, PGY2-4) and number of US studies previously performed (
