Introduction: Psoriasis (PSO), a chronic inflammatory disease associated with an accelerated risk of early MI and adipose tissue dysregulation, provides an ideal clinical model to study inflammatory atherogenesis. Recently, PSO patients were shown to have increased high-risk plaque (HRP) prevalence by CCTA when compared to healthy controls. However, the interrelationship between visceral adiposity and total HRP score in PSO patients has yet to be characterized. Hypothesis: We hypothesized that VAT volume would associate with total HRP score by CCTA in a PSO cohort. Methods: Consecutive PSO patients (n=105) underwent CT and CCTA (Toshiba 320 slice) scans to measure abdominal adiposity volume and characterize HRP morphology respectively. HRP phenotyping was performed using QAngio CT (Medis). HRP was defined as either positive remodeling (index > 1.1) or low attenuation (< 30 HU) and tallied into an HRP score. The relationship between VAT volume and HRP score was analyzed by multivariable linear regression (STATA 12) Results: Patients were middle-aged, mostly male, had moderate PSO severity, and low cardiovascular (CV) disease risk by Framingham risk score (Table 1). Multivariable regression analysis revealed a direct association of VAT with HRP total score beyond adjustment for CV risk factors and systemic/biologic PSO therapy (β=0.31, p=0.03). However, this same relationship was not significant for SAT (β= -0.09, p=0.50). Conclusions: Directly quantified visceral adiposity, but not subcutaneous adiposity, is associated with total HRP score in PSO. These findings suggest that VAT may capture early metabolic risk for HRP development and future cardiovascular events in PSO. Larger prospective studies are needed to confirm these findings.
