Primary purpose of this position The purpose of the Integrated Care Consultant – Commissioned Services role is to support system-wide perspectives and responses to improve health service coordination through the identification of system and service inefficiencies and effectiveness within the North West region of Murray PHN. This position will play a key role in supporting the procurement and commissioning of health services across the North West region of Murray PHN. This position is responsible for the day-to-day engagement with, monitoring of and reporting upon the outcomes of commissioned services. The role will work within a stakeholder management framework and will provide an important interface between the provider Murray PHN. Key responsibilities With guidance from the Executive Director Regional: • Ensure primary health services purchased or commissioned by Murray PHN are delivered within program guidelines and in a way, that is consistent with the values and objectives of Murray PHN. • Support regional team members to implement purchased or commissioned services as required • Monitor and report actual performance against agreed budgets and commissioned services. Make recommendations for unallocated activity within the region and respond to identified issues. • Work with and closely support procured services to deliver expected strategies outlined within the contract assisting in addressing any barriers to program implementation • Implement and monitor business systems to collect accurate data set(s) for program reporting, evaluation and planning. • Contribute to the review and implementation of contract governance and continuous quality improvement activities. • Contribute to the timely completion and submission of reports and plans in accordance with contractual obligations and business needs. • Establish and maintain strong operational and strategic relationships to support the understanding of local issues and the development of effective initiatives. • Utilise internal and external information sources to understand health issues, needs and gaps in a regional context.

• Participate in relevant networks and events to strengthen opportunities for collaboration, service integration and coordination. • Actively manage risks by complying with organisational policies and procedures and escalating risks for higher-level attention when required. • Provide support for providers in program implementation/reporting (for example ITC) and the management of invoices Other responsibilities • Actively participate in Murray PHN committees and working groups. • Work in accordance to and support Murray PHN strategic objectives as directed. • Work in a safe manner, adhering to all work health and safety (WH&S) requirements and report all hazards and incidents through the organisation’s WH&S processes. • Comply with organisational policies and procedures including the Code of Conduct and instrument of delegation. • Actively manage risks by complying with organisational policies and procedures and escalating risks for higher-level attention when required. Note: Some travel may be required as part of this role. Note: this position will require a current driver’s licence which must be provided prior to commencing employment.

Specialist expertise • A degree qualification in a health-related field combined with more than five years’ experience in a similar role. Murray PHN spans the northern region of Victoria. While the majority of the catchment area is located within the State of Victoria, it also includes the twin city of Albury/Wodonga. The estimated resident population is 565,000 people. It is estimated that the Murray PHN area includes: 715 general practitioners 44 acute facilities 200+ pharmacies 150+ residential aged care facilities 9 Primary Care Partnerships 20 Local Government Authorities There are 31 PHNs (Primary Health Networks) across Australia.
